Philam Vitality 100: 100 Garmin Vivo Fit 4 monthly

ELIGIBILITY:

• You must be a policyholder of any Vitality Integrated Product with policy/membership effectivity date of April 1, 2019 to December 1, 2019, with the following criteria:
  1. must be a Philippine citizen or permanent resident of the Philippines,
  2. must be 18 years old and above
• Philam Group employees and advisors who meet the above the criteria, except for Philam Vitality team members, are eligible to join.
   

MECHANICS:

1. You must achieve Silver Philam Vitality Status within the first 90 days of membership effectivity date to be eligible to one (1) raffle entry.
   
   To reach Silver Philam Vitality Status, you must earn at least 10,000 Philam Vitality Points or higher, from the beginning of your membership anniversary date, by participating in points-earning activities of the Philam Vitality program.
   
   
2. There will be 100 Garmin Vivofit 4 fitness devices to give away every month for seven (7) months. The draw dates will be on the 15th of every month starting July 15, 2019 to January 15, 2020.
   
   
3. In case there are less than 100 members who achieved Silver Philam Vitality Status in a given month, all members who achieved the requirement will automatically win the raffle. Prizes that were not given due to lack of winners shall be forfeited in favor of Philam Life and BPI-Philam.
   
   
4. Each monthly draw will get qualified entries from that draw date’s previous three (3) months. Cut-off for each monthly draw is the last day of the draw date’s previous month.
   
   
5. The Head of Philam Vitality with representatives from Philam Life Marketing, Audit, and Compliance will draw winners at the Philam Life Head Office, 18F Net Lima, 5th Ave cor. 26th St., Bonifacio Global City, Taguig City. Announcement of winners will be on the day after the raffle draw through the official social media accounts of Philam Life. Winners will be notified through SMS and registered email.

TERMS AND CONDITIONS:

1. The raffle prize is not convertible to cash. By accepting the prize, winners agree to hold Philam Life and BPI-Philam free and harmless from any liability that may arise from participation in the promo or acceptance and/or use of the prize.
   
   
2. Philam Life and BPI-Philam reserve the right to verify the eligibility of the winning entry, and forfeit the prize if criteria is not met. The winner’s Philam Vitality membership must be active and the membership fee must be fully paid at the time of eligibility.
   
   
3. .As a qualified raffle participant, you agree for Philam Life or BPI-Philam to collect information for purposes of contacting you regarding the raffle promo.
   
   
4. The promo organizer’s decision with regards to the Promo Mechanics, winners and prizes, with the concurrence of the Philam Group Legal Team, is final.
   

CLAIMING AND NOTIFICATION:

1. Raffle draw winners will be notified by Philam Life or BPI-Philam through SMS and registered email within seven (7) days from the raffle date.
   
   
2. The winner shall claim the prize at the Philam Life Head Office, Net Lima Building, 5th Avenue corner 26th Street, Bonifacio Global City, Taguig City. Redemption period shall be within sixty (60) days from notification. If the prize is not claimed within the stipulated period, unclaimed prize will be forfeited in favor of Philam Life or BPI-Philam.
   
   
3. Winners outside Metro Manila may claim their prize in the Philam Life or BPI-Philam branch nearest their registered place of residence from the Philam Life or BPI-Philam representative identified in their notification email.
   
   
4. Upon claiming the prize, winner must present a printed copy of the notification email and two (2) valid forms of identification or government-issued I.D. (SSS, Unified Multi-Purpose ID, Driver’s License, Voter’s ID). If there is any discrepancy between the winning entry and the documents produced for verification, the entry will be deemed disqualified.
   
   
5. Should the winner send a representative to claim the prize, the representative must present an authorization letter from the winner, a printed copy of the notification email, photocopy of the representative and winner’s two (2) valid identification (SSS, Unified Multi-Purpose ID, Driver’s License, Voter’s ID).
   
   
6. By participating and winning in the raffle, you agree to cooperate with, and participate in any advertising or publicity activities of Philam Life or BPI-Philam relating to the promo. You also agree that Philam Life or BPI-Philam may publicize your name and photograph, in such mode and manner as Philam Life or BPI-Philam considers fit. Your name may be published on Philam Life or BPI-Philam's website & social media channels, in such mode and manner as Philam Life or BPI-Philam considers fit.

Reward: Raffle 100 Garmin Vivofit 4 every month (total of 700 fitness devices for 7 months)
