    Philam Vitality 100: Philam Vitality Health Review Reward

    Complete the Philam Vitality Health Review from June 1, 2019 to December 31, 2019 and automatically receive a free reward via the Philam Vitality Active app!

    The Philam Vitality Health Review (VHR) is an in-depth online assessment that lets you discover your Philam Vitality Age and get your personal pathway to better health by sharing your health, lifestyle, behaviors and clinical measures such as height, weight, blood glucose (sugar) and blood pressure.

    ELIGIBILITY:

    • You must be an active Philam Vitality member.
    • You have not completed your Philam Vitality Health Review for the current membership year.

    MECHANICS:

    1. You must complete your registration in ePlan - Philam Life and BPI-Philam's 24/7 online self-service customer portal.

    2. You must download and log in the Philam Vitality Active app.

    3. You must complete the Philam Vitality Health Review either in the Philam Vitality website or the Philam Vitality Active app to become eligible for the VHR reward.

    4. The Philam Vitality Active app will prompt you via the in-app notification that you have earned the VHR reward.

    5. To redeem the reward, you must choose one (1) from the list of the weekly rewards.

    6. You must redeem your reward within two (2) weeks from being qualified for the reward. If you do not use your reward within two weeks, it will automatically be removed from your Rewards Wallet.

    7. You can earn 1,000 Philam Vitality Points for completing the Philam Vitality Health Review.

    8. By joining this promotion, you are agreeing to the Terms and Conditions set out in the official mechanics to be posted on Philam Life and BPI-Philam’s official Facebook pages.

    9. Philam Life or BPI-Philam reserves the right to cancel this promotion and/or amend the terms and conditions without prior notice.

    10. All issues pertaining to this promotion shall be resolved by Philam Life or BPI-Philam.
