    the terms and conditions at a glance

    Insurance coverage takes effect on the effective date stated in the Proof of Cover (24 hours from date of purchase) and terminates exactly four (4) months thereafter. The Proof of Cover (POC) is good for four (4) months for a maximum of five (5) POCs at any given time. In the absence of a designated beneficiary, benefits will be given to the surviving beneficiaries in the preference: the member's:

    (a) widow or widower

    (b) surviving children

    (c ) surviving parents

    (d) surviving brothers and sisters

    (e) executors or administrators
