Here’s a convenient way to file a claim. Simply scan your documents and submit the requirements online wherever you are with eSubmission.

Claims requirements

We’re here with you on your road to recovery. To start your claim request, please prepare clear copies of the requirements below:

1. Valid Government ID
2. Attending Physician's Statement (APS)
3. Police or Narration Report
4. Official Receipts (OR)

3. Hospital Statement of Account (SOA)

We express our deepest sympathies for your loss. To start your claim request, please prepare clear copies of the requirements below:

1. For the Insured:

  • Death Certificate of the Deceased
  • Police Investigation Report (If the cause of death is due to an accident)

2. For the Beneficiary

  • Valid Government ID
  • Marriage Contract (If the Beneficiary is the spouse)
  • Birth Certificate (If the Beneficiary is below 18 years old)
