Bank account should be in the same currency as the enrolled policy and not an "AND" or joint account.
Mobile number must be active to receive the reference number for pick-up of benefit.
Policyowner must present a valid government ID upon pick-up of benefit.
If benefit is more than P500,000.00 or $10,000.00, please enrol to Credit to Bank Account in compliance with Anti-Money Laundering Act (AMLA) regulations.

I/we agree for the Company to use my information and all related

I/We am/are assured that this will not affect the Company's ability to provide quality service in relation to my/our existing policies.
I certify that I am the Policy Owner of the policy with BPI AIA and
that I am the Account Owner of the aforementioned Bank Account. I
acknowledge that the credit or deposit by BPI AIA of the benefit or
amount due to me to the bank account I have designated above shall
release and forever discharge BPI AIA of and from all actions, claims
and demands on all matters involving said benefit or amount. Further,
I certify the correctness and accuracy of the above information I
provided BPI AIA and that any discrepancy may cause delay in the
crediting of proceeds to my account. BPI AIA will be informed in
writing of the future changes of the above information.
