If the insured undergoes a medically necessary surgery, we will pay an additional benefit equal to the amount stated in your policy to the policy owner.

• Operating Room Report of the Surgery/ Operative Technique Incident Report/ Narrative/ Police Report
• All medical records related to the claim (Please refer to Additional Requirements)
